Insurance Requirements for ABA Therapy Authorizations

Commercial health plans may require documentation before approving ABA therapy. Requirements vary by carrier and plan but may include:

  • Autism diagnosis: Documentation of a confirmed autism spectrum disorder diagnosis may be required.

  • Recommendation for ABA therapy: Some plans may request documentation from an appropriate healthcare professional recommending ABA services.

  • Prior authorization: Many plans require authorization before an assessment or ongoing ABA services can begin.

  • Clinical documentation: Treatment plans, assessments, progress information, and other records may be required as part of the authorization process.

Because requirements vary, families should confirm their plan’s current ABA benefit and authorization requirements directly with the insurance carrier.

In-Network (INN) vs. Out-of-Network (OON)

A provider’s network status can affect both access to care and what a family may pay.

In-Network (INN)

An in-network provider has a contract with the health plan to provide covered services according to that plan’s requirements and negotiated rates.

Depending on the plan, families may have lower out-of-pocket costs when using in-network providers.

Out-of-Network (OON)

An out-of-network provider does not have a contract with the health plan.

Some commercial plans include out-of-network benefits, while others do not. When out-of-network coverage is available, families may have a higher deductible, coinsurance, or other financial responsibility.

Families should verify whether ABA therapy is covered out of network and how their plan handles reimbursement before beginning services.

Key Insurance Terms

Insurance benefits and cost-sharing requirements can vary by health plan and may change when a plan renews. Understanding common insurance terms can help families better estimate the potential costs associated with ABA therapy.

Terms to be aware of:

  • Copay: A fixed amount you may pay for a covered healthcare service. For example, your plan might require a $25 copay per visit or service, depending on how ABA therapy benefits are structured.

  • Deductible: The amount you generally pay for covered healthcare services before your insurance plan begins paying its share. For example, if your plan has a $2,000 deductible, you may be responsible for eligible healthcare costs until that deductible is met, subject to your plan’s specific rules.

  • Coinsurance: The percentage of the allowed cost of a covered service that you may be responsible for after applicable deductibles are met. For example, with an 80/20 coinsurance arrangement, the insurance plan may pay 80% of the allowed cost while you are responsible for the remaining 20%.

  • MOOP (Maximum Out-of-Pocket): The maximum amount you may be required to pay during a plan year for certain covered, in-network healthcare services. For example, if your plan has an $8,000 out-of-pocket maximum, eligible deductibles, copays, and coinsurance may count toward that amount. Once the applicable maximum is reached, the plan generally pays 100% of additional covered, in-network services for the remainder of the plan year. Out-of-network or non-covered expenses may not count toward this limit.

These examples are for illustration only. Families should review their individual plan documents or contact their insurance carrier to confirm how copays, deductibles, coinsurance, and out-of-pocket limits apply to ABA therapy.

Ongoing Requirements after Initial Authorization

  • Insurance authorization does not necessarily mean that the same services or hours will remain approved indefinitely.

Depending on the health plan, ongoing requirements may include:

    • Periodic reassessments: The BCBA may update the assessment and treatment plan at intervals required by the insurer.

    • Family or caregiver involvement: Some plans may require documentation related to caregiver participation or training.

    • Direct care authorization: Recommended treatment hours are based on the child’s clinical needs and treatment plan and may also require approval from the insurance carrier.

    • BCBA supervision: BCBAs supervise treatment, review progress, and make clinical adjustments. The exact supervision and authorization requirements may vary by plan and treatment model.

Families should avoid assuming that a particular reassessment schedule, number of therapy hours, or supervision ratio applies to every insurance plan.

Navigating Insurance Challenges

  • Insurance requirements can differ substantially between plans, even when the insurance company is the same.

    A few useful steps include:

    • Confirm your benefits: Ask specifically whether ABA therapy is covered and whether prior authorization is required.

    • Verify network status: Confirm whether the ABA provider is considered in network under your specific plan.

    • Understand your costs: Ask about deductibles, copays, coinsurance, and out-of-pocket limits.

    • Review your plan documents: Your Summary of Benefits and Coverage or plan handbook may contain important details.

    • Ask whether your employer plan is fully insured or self-funded: This distinction can affect which state insurance requirements apply.

In New Jersey, state-mandated benefits generally apply to health insurance policies issued in New Jersey, while self-funded employer plans are governed differently and are not necessarily subject to the same state mandates.

Additional Tips

Insurance benefits and authorization requirements can change over time.

Review your coverage when your plan renews and notify your ABA provider if your insurance carrier, policy, or member information changes. Changes in coverage may require new benefit verification or authorization before services can continue.

If you are unsure about your benefits, contact the number on your insurance card and ask specifically about coverage for Applied Behavior Analysis for autism spectrum disorder.
